I (Owner of Equine) Here by release Samuel Kaufman of Circle K Farms from any and all liability that may result from any injury sustained by or to:

I further agree to hold Samuel Kaufman, his family and Circle K Farms harmless from any judgement for damages resulting from any injury, damages or death as a result of Training and Boarding.

I (Owner of Equine) also understand and agree that all horses must be up to date on dental work and have their wolf teeth removed prior to dropping off for training.

I also understand and agree that if my horse arrives for training with wolf teeth that they will be taken out by a equine dentist at my (owner of equine) expense. *
